I offer personal one-on-one home visits with either a comprehensive written assessment
or an in-home hourly assessment. I also offer monthly monitoring, escort to medical appointments, fixing problems
with benefits and eligibility, locating favorable facilities when necessary, monthly bill paying, the location of caregivers
and physicians, and most of all some compassion when life's challenges present themselves. Hourly rate for care management, money management, and all other services..................................................$60.00
per hour Written Comprehensive Assessment with Care Plan: Individual....................................................................$150.00
Couples in the same residence......................................$225.00 Telephone Calls will be charged after 15 minutes at $1.00 per minute. All invoices will be payable if the funds are available at the time of receipt of services.
Otherwise payments are due within 20 days from date of invoice. |
